How do I schedule an appointment?

You can schedule a visit by clicking here. All of our appointments are virtual at this time. We ask that you fill out the pre-screening form. If we feel that we cannot fulfill your needs, we can give you referrals to other providers in the community.

What should I expect during the first consultation?

We will spend our first visit getting to know each other and making sure that we are the right fit for treatment together. We will discuss your concerns, past treatment, and your goals for treatment before developing our treatment plan.

I think I have ADHD and want to get started on medication. Can you help?

For ADHD evaluations, I work on trying to better understand childhood behaviors (as it is a major requirement for the diagnosis) and often seek collateral information from parents or others to help further identify these struggles. If that is unavailable or inconclusive, we may consider further neuropsychological testing (performed by a neuropsychologist) to further characterize your struggles. As many ADHD medications can be addictive, I generally do not start these medications during the first visit.

Are there any patients that may not be a good fit for your practice

Patients requiring frequent crisis management or after-hours monitoring are often better served in a practice that offers more wrap-around services (such as an academic medical center or community mental health clinic). Many severe eating disorders, substance use disorders, and personality disorders require more intensive treatment team management than offered at our clinic.

What is the typical frequency of follow-ups for your patients?

We tend to see patients with active medication adjustments or symptom instability every 4-6 weeks (or more frequently if clinically necessary). Once a patient is stable, we will follow up every 3-4 months.

Are you part of any insurance plan?

We currently accept the Blue Cross Blue Shield PPO and Blue Choice PPO plans.

What does it mean to be out-of-network?

This means that we are not contracted with your insurance company, but we can give you a “superbill” that you can submit for reimbursement to your insurance company. Please check with your insurance company first to get an estimate of reimbursement. Medicare and Medicaid do not offer any out-of-network reimbursement.

Are there any restrictions to telemedicine?

Due to the Ryan Haight Act, we are unable to prescribe controlled substances, such as benzodiazepines, stimulants, and sleep medications, without an in-person visit at least once every two years. We can only see patients who are physically located in Illinois at the time of the visit.

How do I avoid a No Show fee?

We ask for a 24-hour cancellation notice for follow-ups to avoid incurring a no-show fee.

How can I get a copy of my records or have them forwarded to another provider?

We just need you to sign a release of information form found here.

Do you see patients under 18?

At this time, we are not able to see minors.

Do you write letters for emotional support animals?

Generally, I do not write ESA letters except in a few rare situations.
