PARKING
There are two dedicated spots available for patients on the end of the parking lot behind (entrance to the loft is on the east side of the building) of the building on Armitage Ave. There is also street parking available if those spots are full.
This form should NOT be used for emergent or urgent issues. If you have an emergency please call 911 or go to your nearest emergency room. For your privacy, do not include private medical information.
