• NEW PATIENT REQUESTS: I am accepting limited new patients at this time. You can request an appointment with the form below. Once we receive your request, we will review it and send an email notification if we can accommodate your request.

• WHAT TO DO BEFORE YOUR FIRST VISIT:

  • After your appointment is accepted, you will receive an email invite from Charm to create a profile for the new patient portal. If you do not get this, please message or call me (you can send a message to the contact us section of this website).

  • Once you create your patient portal, go to “Questionnaires,” you will have access to the assigned patient questionnaires (they are different from the pre-screener form).

  • Remember to complete the questionnaires (available after signing into the patient portal) for new patient consultations before the visit. If the questionnaires are not filled out at least 5 minutes before your scheduled visit, your appointment will be canceled and need to be rescheduled.

  • If this is an evaluation related to ADHD, please forward a copy of your prior psychiatric records or neuropsychological testing to me via fax (312-872-7660) or a message in the patient portal.
